    All students who plan on participating in a school sanctioned athletic event must have a physical on file. The physical must be dated between May 1 and the students first practice in preparation for interscholastic athletic participation. The student shall have a physical examination by or shall provide certification from a physician holding an unlimited license to practice medicine. The state requires that the physical be signed by a licensed medical doctor or doctor of osteopathic medicine (D.O.). This excludes Nurse Practitioners, Physician Assistants and Chiropractors.

    Currently, the team physician conducts reduced rate physicals for Hobart athletes. His office establishes the date and time. This information will be reported to the local newspapers and you can check the website for the time and dates.

    Also included in the physical form is the consent form. The consent form is for the student and the parent to complete.
